If he did not go… He would still be here… My experience, only two months ago, with this rehabilitation facility was devastating, heartbreaking , Unimaginable and traumatic and I feel obligated to share what happened to my father. For approximately two weeks, I made daily requests expressing my concerns that my father was not receiving adequate food, fluids, or the rehabilitation care he needed. During this time, the nurse repeatedly told me, “He is not eating or drinking.” Instead of taking meaningful action to address why he was not eating or drinking or ensuring he received the nutrition, hydration, and medical attention he needed, I watched his condition continue to decline. The conditions of my father’s room were also unacceptable. On multiple occasions, I observed the room to be dirty and unsanitary. Nursing trash was left on the floor, his bedding was soiled, the room was excessively hot, and he was frequently left uncovered without his bed blankets. Seeing him lying exposed and uncomfortable while already in such a weakened condition was heartbreaking. When I was told the facility would “contact the doctor” about my concerns, I insisted that my father needed immediate emergency medical care. I told the staff that if they did not arrange for his transfer immediately, I would call 911 and have the Holmdel Police Department assist in getting him to the hospital. Within about two hours, my father was transported to Riverview Medical Center. There, he was diagnosed with severe dehydration, malnutrition, kidney failure, and heart failure. He was so critically ill that he was never returned to the rehabilitation facility. Five days later, my father passed away in the hospital. I was at his bedside, holding his hand, watching the heart monitor drop to zero as he took his last breath. About an hour before he died, he was still asking me for a cup of cold soda with ice and a smile. The grief of losing my father will stay with me forever. I believe my repeated concerns over those two weeks were ignored, despite my daily requests for help. Hearing over and over that “he is not eating or drinking” was not an acceptable response when he clearly needed intervention. I trusted this facility to provide my father with proper nutrition, hydration, rehabilitation, a clean and safe environment, and appropriate medical care. In my experience, that trust was not honored. • Psalm 139:7–12 – emphasis: there is nowhere a person can go to escape God’s presence or sight

This place is horrible. I was there for a week for rehabilitation. I wanted to leave and they wouldn't let me leave like they were holding me hostage. Also I tried to get up and I fell on the floor I' rang the bell, I was on the floor for quite some time before somebody came to help me Keep your family members out of there. Look for another place and the workers are not professional., also the day I got there I wanted a cigarette and they told me no smoking on a premise that's understandable, well, so I tried to get out of bed that evening when I got there, and I had one of the aides pushed me in the corner, that was never reported that was a hush. This home help facility is a nightmare

I’m writing this review with deep frustration and concern about the horrific care my brother received at this rehabilitation facility. From the moment he arrived, the level of neglect was appalling and completely unacceptable. My brother was admitted with multiple traumatic injuries — a broken pelvis, a femur broken in three places, a broken shoulder, and a fractured lower back. Despite the severity of his condition, he was placed on deflated and broken air mattresses — it took five separate mattresses before he was given one that actually worked. Even then, it was so inadequate that our family had to buy a mattress topper ourselves to help ease his pain. Medication administration was a nightmare. He was regularly left waiting hours for pain relief, and when he called for nurses, they often took an extremely long time to respond — or didn’t show up at all. To make matters worse, one of his prescribed medications was completely overlooked, and he still hasn’t been put back on it. This kind of oversight is not just careless — it’s dangerous. Hygiene and cleanliness were equally unacceptable. Showering was rare, even when requested. The facility was filthy, with a used bedside commode left in his room for long periods until we repeatedly asked for it to be cleaned. My mother had to come every day to change his sheets herself, because the staff simply wouldn’t. And after all this, when he was finally discharged, he was sent home without any of the necessary medical equipment. No wheelchair. No walker. No commode. Nothing. We had to buy all of it out of pocket, and the equipment the facility was supposed to provide didn’t show up until a full week later. This was hands down the most unprofessional and neglectful facility I’ve ever encountered. The level of disregard for patient safety, comfort, and basic dignity is beyond alarming. I wouldn’t recommend this place to anyone, and I sincerely hope this review leads to real accountability and change before more patients are harmed.
